The inspector general of the U.S. Department of Veterans Affairs is investigating mistakes at the Buffalo VA Hospital.
Inspector General George Opfer will conduct a review to determine how the hospital broke protocol involving the re-use insulin pens, which possibly exposed more than 700 patients to HIV and hepatitis B and hepatitis C.
Opfer has written to Senator Charles Schumer and Buffalo Congressman Brian Higgins to inform them of the probe, and both lawmakers say they are pleased that a review is underway.
