The federal Food and Drug Administration issued a directive in 2009 for hospitals not to reuse insulin pens on multiple patients.

The directive came more than a year before the Buffalo VA Medical Center began reusing insulin pens on hundreds of patients, exposing many to possible HIV and hepatitis infection.

Congressman Chris Collins says the mistakes are unacceptable, and is demanding to know how the hospital came to ignore the directive.
